What to Bring
Please bring a valid photo ID, your insurance card (if applicable), and a list of any current medications. If you completed your forms online, you’re all set—we’ll have them ready. Have relevant dental or medical history on hand so we can tailor your care to you.

We’re in-network with Delta Dental Premier and accept most other insurance plans. Our front office team is here to answer questions, help you estimate out-of-pocket costs, and file claims on your behalf so you can focus on your care.
